Measurement is clinical data, not decoration

A useful measurement allows you to compare, follow up, categorize or communicate risk. An isolated measurement can create false precision. Therefore, the report must record what was measured, where, in which axis, with which unit and within which scope.

In a flow with assistive AI, every measurement needs to be checked against imaging, worksheet or validated dictation. The system can format the text, but should not invent numbers, swap units or assume comparison.
